Students at Hoosick Falls CSD will be asked to complete a Health Screening on a daily basis to assist in identifying symptoms and cases of COVID-19. Parents/Guardians are receiving a link to the Daily Student Health Screening in their email and/or as a text message. This link will bring you to a three-question form that can be filled out by simply choosing ‘yes’ or ‘no’ to each question. The three questions are:
- Have you traveled internationally or to one of the following states in the past 14 days: The list of states on New York State Travel Ban are then listed.
- Have you knowingly been in close or proximate contact in the past 14 days with anyone who has tested positive for COVID-19?
- Have you tested positive for COVID-19 in the past 14-days?
Every student has their temperature on the bus or when they are dropped off at school each morning. If their temperature is 100.0°F or above, they will not be allowed to enter the school building.
We are asking that the screening be completed by 9:00 a.m. each school day throughout the year for all in-person, hybrid and 100% remote students.
Some parents/guardians may receive both an email and text or both parents/guardians may receive the same email and/or text to complete the screening. The system will not allow double entries. Each screening may only be completed once per student per day.
The system also sends out a series of reminders each morning if the screening has not been completed for a student. These will stop once the form is complete.
If you have not received an email or text for your child, please contact the Elementary School Office or High School Office to update your information in our system.
